Establishment Inspection Trends

All registered veterinary establishments are subject to routine inspections by the Department of Health Professions. See below for a refresher on commonly overlooked establishment requirements found in recent inspections.

Expired Drugs
Expired medications in working stock, including injectable drugs that lack the first use or first puncture date, resulting in the inability to determine the expiration date, and expired drugs used in euthanasia procedures. Regulations require expired drugs to be removed from working stock immediately upon expiration, including vaccines. 18 VAC 150-20-190(G) of the Regulations.

Distribution Log
Common items include missing waste entries, missing authorizing veterinarian identification, missing drug strength, and unclear documentation of whether drugs were administered or dispensed. Distribution logs for Schedules II–V drugs must include full date of the transaction, drug name, strength, and amount dispensed, administered, and wasted, owner and animal identification, and the identification of the veterinarian authorizing the administration or dispensing of the drug. Virginia Code § 54.1-3404(D) and 18 VAC 150-20-190(H) of the Regulations.

Biennial & Change of Veterinarian-in-Charge Inventories
Incomplete or missing biennial and change of veterinarian-in-charge (“VIC”) controlled substance inventories, absence of VIC signatures, lack of notation indicating whether the inventory was taken at opening or closing of business, and failure to include expired drugs awaiting destruction. Change of VIC inventories must be conducted on the effective date of change of the VIC. Biennial and change of VIC inventories must be signed by the VIC and maintained onsite for two years. Inventories of Schedule II drugs must be separate from Schedule III-V drug inventories. Virginia Code § 54.1-3404(B), 18 VAC 150-20-181(A)(3) and (B)(4), 18 VAC 150-20-190(J) and (K) of the Regulations.

Missing Information on Reconstituted, Compounded, or Prepackaged Drugs
Records for bulk preparation missing required details such as drug name, strength, quantity prepared, lot/control numbers, expiration dates, and verifying veterinarian initials. Transfer or prepackaging requires full labeling and recordkeeping, with expiration dates based on the original stock or one year—whichever is sooner. 18 VAC 150-20-190(M) of the Regulations.

Nonfunctional Surgical Emergency Lighting
Automatic surgical lighting systems with inoperative battery backup. Emergency lighting must activate automatically upon power failure and provide sufficient illumination to complete or safely stabilize a surgical procedure. 18 VAC 150-20-200(A)(2)(c)(6) of the Regulations.
